Can't convert ACT! 2000 contacts to Outlook

I am trying to import my ACT! 2000 (version 5.0) Contacts and Notes into Outlook 2003. The import wizard in Outlook shows it can import ACT! files, but the import wizard doesn't recognize my ACT! database file. Does anyone have any suggestions??? I tried to export the contacts out of ACT! as a .csv file. It exported, but left out all the Notes. I have to have the Notes in Outlook.
