Internet Connection

When I attempt to dial up, my username and phone access number gets reset to an unknown entry…I also assume that it's resetting my password. I am still able to go in and manually re-enter. I have ad-aware, Spybot S &D, and AVG running, and I also scanned with Hijack this. Here's my log file, could you please tell me what to get rid of? Thanks.
