HELP! Can't remove 504.filost bug

Hi!
I've been trying in vain to remove this 504.filost bug but have been unsuccessful so far- it's driving me CRAZY! I'm hoping your thread is still open!
Have run the usual Spybot, Adaware, AVG etc checks but was hoping some kind hearted soul out there could check out my Hijackthis log for me to see if it's gone or advise me how to get shot of this nuisance!
