Hi outlawRB. Sorry for the delay. Flat out here :).

Hi. You are running hijackthis from a temporary folder, can you please download the self-extracting zip version from here. You can move it into it's own folder too, if you wish.
Uninstall the other version first, then manually delete the file. Scan with hijackthis and tick the boxes next to all the following entries, then close all browser and explorer windows, and hit the "Fix checked" button.

Cannot remember telling you to put that on  but if you go to the folder that you unzipped it to, you will see an reg file with part of the file name being uninstall. Got a screen shot for you.

outlawRB, you sent a reopen request for this thread but I will leave that one closed as you are receiving help here.

Regarding your IESpyads problem, this quote from the developers site:

Note: The uninstaller (IE-ADS-UNINST.REG) and the installation utility (INSTALL.BAT) included with every version of IE-SPYAD can be used to uninstall both the current version of IE-SPYAD as well as any previous version you have. For more information, check the ReadMe.txt.

makes it clear that a fresh copy can be downloaded and then just run the uninstaller from that copy.

IESpyAds is a passive tool, it simply puts a list of sites in the restricted zones area of the registry. Unless you frequent one of the sites on the list (which you probably shouldn't do, given the fact that they are on the list because of malicious content) there should be no impact on your surfing at all. To find out what sites are in the restricted zone go to Start>Control Panel>Internet Options, click the Security tab then the Restricted Sites icon, next click the Sites button and a dialog will open with a list of the blocked sites. Scroll through the list and see if any sites you use are on it. They are listed by domain, for example , in the following url the domain is in bold. You can remove a site from the list with the Remove button.

http: //forums.tomcoyote.org/index.php?

On a block list, it would look similar to this:
*..tomcoyote.org

If that domain were on the list you would still be able to go to the site but some content would be blocked, according to your restricted zone settings.
