Yes, this does appear to be a hardware issue and a reformat would be in order.
If you wish assistance with this, please post in our Hardware Forum and the tech's will assist you with the process.

It took several restore points back, not just the one made by the tool. It's behaving like it did before I started this process. This is an interesting learning experience. My plan is to remove the HD and scan it as a secondary drive in a different computer. That can sometimes find things that are hidden by clever malware.
