I've spent 2 weeks trying to set up a website based order form,. I need the form set up so that people can add to each named column how many of each ite they need etc. then they need to be able to submit the completed order form to me via email from the website.

I have attached the original order form:Attached File  PAPERWORK_ORDER_FORM.doc ( 44.5K ) Number of downloads: 7


Hope I did it right?
